Getting Started

Four simple steps to services

1

Contact PerformCare

PerformCare is the CSOC access point. Call 1-877-652-7624 to ask about appropriate services, eligibility, and authorization.

2

Ask About Appropriate Services

PerformCare may recommend or authorize a Biopsychosocial Assessment. The assessment is conditional; it is not required for every caller.

3

Discuss Provider Options

For an assessment, ask PerformCare whether Boundless is available. For authorized ongoing services, ask an existing care manager. A request does not guarantee assignment, availability, or approval.

4

Optional Boundless Questions

Contacting Boundless first is optional. We can answer general questions, but a website inquiry does not authorize care.

Contacting Boundless first is optional. Call us for general questions, help understanding the process, or help preparing for a PerformCare call. PerformCare remains the CSOC access point and determines eligibility and authorization.
